The Horse's Breath Under the Microscope – What Does Mucus Analysis Reveal?

We've all been there: your horse is coughing, seems lethargic, or isn't performing as well as they used to. We try teas, syrups, and hope for improvement. But what if the cause lies deeper – deep within the lungs of our four-legged friend? Sometimes we need to look closer to understand what is really going on. One of the most precise diagnostic methods is Bronchoalveolar Lavage, or BAL for short. Sounds complicated? It’s not at all. Let’s take a look through the microscope together.

What exactly is Bronchoalveolar Lavage (BAL)?

Think of BAL as a kind of "lung rinse" for diagnostic purposes. It is a veterinary procedure that allows us direct insight into the smallest airways, the alveoli (air sacs). This is exactly where vital gas exchange takes place and where inflammation occurs in respiratory diseases like Equine Asthma. A simple nasal swab or blood analysis cannot provide this specific information. BAL is the gold standard for collecting cells and secretions directly from the "scene of the action."

How is the examination performed? Don’t worry, it’s a routine procedure!

Many owners get worried when they hear the word "procedure." However, BAL is an established and well-tolerated method for horses. Usually, the horse is lightly sedated to remain relaxed. The veterinarian then guides a thin, flexible tube (an endoscope) through a nostril deep into a section of the lung. A small amount of sterile saline solution is flushed into the lung through this tube and immediately suctioned back out. This recovered fluid – the lavage – now contains a valuable mixture of mucus and cells found in the deep airways. The whole process usually takes only a few minutes, and most horses handle it without any problems.

Laboratory Analysis: What the cells tell us

The recovered fluid sample is the key to diagnosis. It is sent to a specialized laboratory for cytological examination. This means an expert counts and identifies the different cell types under a microscope. The composition of this "cell cocktail" tells a precise story about the health status of the lungs:

  • Macrophages: These are the "cleaning cells" of the lungs. Their job is to remove dust, pollen, and other foreign bodies. In a healthy lung, they make up by far the largest proportion of cells (over 60%).

  • Neutrophils: These cells are the immune system's "first responders" to inflammation. A significantly increased number of neutrophils (normally below 5-10%) is a clear alarm signal for active inflammation. A very high percentage is typical for severe Equine Asthma (formerly known as RAO or heaves).

  • Mast Cells: They play a central role in allergic reactions. Increased levels (normally below 2%) often indicate a milder form of Equine Asthma triggered by allergens like mold spores in hay or dust.

  • Eosinophils: These cells are also part of the allergic defense system. An increase can also point toward an allergic component, similar to mast cells.

From findings to targeted therapy

What does this mean for you as an owner? Quite simply: BAL results enable your veterinarian to make a precise diagnosis. Instead of just treating "some kind of cough," we now know exactly what is happening in the lungs.

  • High Neutrophil count? We are dealing with severe inflammation. Anti-inflammatory medications (often administered via an inhaler) and a radical improvement in stable hygiene (dust and ammonia reduction) are the right way to go.

  • High Mast Cell count? The focus is on an allergic reaction. In addition to treating inflammation, managing allergens is crucial: steaming or soaking hay, using low-dust bedding, and ensuring maximum fresh air supply.

BAL takes away the guesswork and gives us a clear roadmap. It helps in choosing the right treatment, avoiding unnecessary medication (such as antibiotics for non-bacterial inflammation), and monitoring the success of the therapy. If your horse shows signs of respiratory issues, talk to your vet about diagnostic options. A close look into the lungs via BAL can be the first and most important step toward free breathing and a healthy, high-performing horse.
